Eligibility

dependents covered by these plans when employee enrolls.

Disability:

Accrual begins first day of month following date of hire.

Tuition Reimbursement:
After 6 months of continuous employment, Carvel will pay for your tuition based on a grade C or better. Maximum $5,000 per calendar year. Can be used toward the completion of a degree in any field, not just one pertaining to your particular job.

United Healthcare – Point of Service Plan (POS)
In-Network
$15 Copay for most services. Your out-of-pocket expenses are limited. No deductibles or coinsurance. Prescription copays - $10 Generic, $25 Brand, $40 Non-formulary.

POS - Out of Network
Annual Deductible: $1000 Individual, $2000 Family.
Coinsurance Limit: 20% after deductible. Plan pays 80%.
Out-of-Pocket Maximums: $5000 Individual; $10,000 Family.

PPO - In-Network
Applies only to residents of SC; NC; Tallahassee, FL; Alabama; Wisconsin; and Pennsylvania
Higher benefit level when In-network providers are used. Office visits covered at 100% after Co-pay. Prescription Co-pays: $10 Generic, $25 Brand, $40 Non-Formulary

Out-of-Pocket Maximums: None

PPO - Out-of-Network
Annual Deductible: $250 Individual, $750 Family.
Coinsurance Limit: 20% after deductible. Plan pays 80%.
Out-of-Pocket Maximum: $5000 Individual, $10,000 Family.

Both full-time and part-time employees are eligible for medical benefits.

Aetna (MA Referred) Quality Point-of-Service Flex HMO
Office visit co-pays $20.00 per visit; Outpatient surgery will now be subject to a co-pay of $100.00 per occurrence. Inpatient admission’s have a co-pay of $240.00, Emergency room co-pay $50.00, but waived if patient is admitted to hospital. Prescription co-pays- $15 generic, $20 Brand Name Formulary, and $35 for Brand Name Non-Formulary.

CT No referred
Deductible $1000.00 per individual to a max of $3000.00 for family. Primary Care Physician Visits and Hospitalization paid at 70% after deductible.

Dental Benefits

Delta Dental Plan
$50 individual deductible per person per calendar year. $150 maximum family deductible per calendar year. Services covered at:
Preventive - 100%
Remaining Basic - 80%
Orthodontic - 50%
Plan Maximums:
$1,500 per person per year. $1,500 lifetime maximum per person for orthodontia.

Savings Incentive Plan (401K)
Must be at least 21 years of age with 6 months of continuous service. Full-time and part-time employees are eligible.

Employee Contributions:
Minimum of 1% to maximum allowed by law. Highly compensated employees are limited.

Company Contributions:
Carvel will match 50% of the first 6% of income saved. Contributions are fully vested immediately.

Investment Funds:
47 investment choices. Refer to enrollment packet.

Additional Benefits

Vision Discount Plan
A savings of 20% to 60% on eyeglasses, 20% on contact lenses, and 10% on disposable contacts, a minimum of $500.00 on laser corrective surgery.

Flexible Spending Accounts
Annual Maximums: Health Care up to $3,000; Dependent Care up to $5,000.

Life and Accident Benefits
At no cost to the employee. $50,000 Life Insurance Policy.
$50,000 AD&D Policy. May purchase additional life insurance.

Supplemental Life Insurance
Can be purchased in increments of $50,000.00. Employee's contribution varies pending age.

Disability
Short-term & Long-term: Employee receives 60% of monthly
earnings up to a maximum of $10,000.
